Website Privacy Policy
How this website collects, uses and shares information, and the choices you have.
Effective: October 1, 2026 · Last updated: October 1, 2026
1. About this policy
This policy explains how Veridale Psychiatric Nursing and Behavioral Health, PC (“Veridale Health”, “we”, “us”) handles information collected through this website, veridalehealth.com.
This policy covers the website only. It does not cover your medical records or information you share as part of your care, such as during appointments, telehealth visits or through our booking provider. That information is handled under the laws that apply to healthcare providers in California, including the California Confidentiality of Medical Information Act.
2. Information we collect
Information you give us
| Where | What we collect | Why |
|---|---|---|
| Contact form (Contact page) | First and last name, email address, phone number (optional), and your message. | To reply to your question. |
| Review form (home page) | Star rating, name or initials, email address, review headline, review text, and your permission to publish. | To read your review and, if you gave permission, publish your rating, name or initials, headline and review on this website. We never publish your email address. |
Sending a review tells us that you have received care from Veridale Health. Please only send one if you are comfortable with that.
We also receive a hidden “spam trap” field that people leave empty. It helps us block automated spam.
Information collected automatically
- Server logs. Like most websites, our web host records technical details of each visit: your IP address, the page you asked for, the date and time, your browser type and the referring page. Our form handler also keeps a one-way hash of your IP address for a short time to stop the forms being abused (see Security).
- Web fonts. Our pages load fonts from Google Fonts and Fontshare. To send the font files, these services receive your IP address, browser details and the address of our website (not the page you are on).
We do not use analytics, advertising pixels, session recording or similar tracking tools.
Sources
We collect information directly from you when you use our forms, and automatically from your browser when you visit the site.
3. How we use information
- To reply to your messages and questions.
- To review and, with your permission, publish reviews.
- To keep the website working, secure and free of spam and abuse.
- To meet legal obligations.
We do not use website information for marketing, and we do not add you to a mailing list.
4. Who receives information
We only share information with service providers that help us run the website, and only for that purpose:
- Our web hosting provider runs this website and the form handler that receives your message.
- FormSubmit (FormSubmit privacy policy (opens in a new tab)) is the form relay service that sends messages from our contact and review forms to our practice by email.
- Our email provider delivers and stores form messages in our practice mailbox.
- Google Fonts (Google privacy policy (opens in a new tab)) and Fontshare (Fontshare privacy policy (opens in a new tab)) receive the technical details described above when they serve fonts.
We may also disclose information if the law requires it, or to protect the safety of any person.
Links to other services
Our site links to Zocdoc (booking), WhatsApp (chat), Facebook and Google Maps. These are links only: none of these services is loaded on our pages, and they receive nothing from this website unless you choose to follow the link. Once you do, their own privacy policies apply. WhatsApp is not intended for sharing health details with us.
5. No sale, sharing or targeted advertising
We do not sell personal information, and we do not share it for cross-context behavioral (targeted) advertising. We have not done so in the past 12 months. We do not share personal information with third parties for their own direct marketing.
Global Privacy Control and Do Not Track. Because we do not sell or share personal information or track you across websites, these browser signals do not currently change how this site behaves. If your browser sends a Global Privacy Control signal, our home page treats optional information collection as off unless you choose to allow it. If our practices change, we will update this policy first and honor opt-out preference signals as the law requires.
6. Cookies and tracking
This website does not set cookies, and we do not use analytics or other optional tracking tools.
Your choices. When you first visit our home page, we ask whether you accept optional cookies and whether you allow us to collect information about how the website is used. You can accept or reject each one. We save your answers, the date and a version number in your browser’s local storage under the name veridale-privacy-choices. It contains no personal details and is never sent to us. You can change or withdraw your answers at any time using the Privacy choices link at the bottom of every page, or by clearing this website’s data in your browser settings.
Because we don’t use optional cookies or analytics today, choosing “Accept all” or “Allow” does not turn anything on at the moment. If we add such tools, they will load only for visitors who said yes, and we will update this policy first.
The light and dark theme switch is not remembered between pages.
7. Health information
Please do not send medical, psychiatric, medication, insurance or other health details through this website’s forms or by WhatsApp. They are meant for general questions only. To talk about your care, please call us at (209) 962-2081.
If you include health information anyway, we use it only to reply to you. We do not use it to draw conclusions about you, for marketing, or for any other purpose.
8. How long we keep information
We keep form messages and reviews only as long as we need them for the purposes above, or as long as the law requires. The form handler keeps hashed IP addresses for no longer than one hour. Published reviews stay on the site until we remove them or you ask us to.
9. Security
Our website is served only over encrypted HTTPS connections, and form messages are sent over HTTPS to our own server. The form handler checks that requests come from our website, limits how often messages can be sent, and does not keep a copy of your message after emailing it to us. Even so, no website or email system is completely secure, which is another reason not to include health details in a form.
10. California privacy rights
Depending on the laws that apply to us and to your information, California residents may have the right to:
- Know what personal information we have collected about you, where it came from, why we use it and who we disclosed it to;
- Access a copy of that personal information;
- Delete personal information we collected from you;
- Correct inaccurate personal information;
- Opt out of the sale or sharing of personal information. We do not sell or share it (see section 5);
- Limit the use of sensitive personal information. We use any sensitive information only to respond to you (see section 7);
- Not be discriminated against for exercising any of these rights.
Some information, such as medical information covered by healthcare privacy laws, is handled under those laws instead.
11. How to make a request
To make a request, email enquiries@veridalehealth.com with the subject “Privacy request”, or call (209) 962-2081. Tell us what you are asking for. Please do not send health details or identity documents unless we ask.
- Verification. To protect you, we will confirm your identity before acting, usually by matching the email address or phone number you used with us. We may ask for more information if needed, and will use it only to verify your request.
- Authorized agents. Someone you authorize can make a request for you. We may ask for your signed permission, and ask you to verify your identity directly with us.
- Response. We will confirm that we received your request and respond within the time the law requires. If we cannot fully meet a request, we will explain why.
12. Children
Our practice cares for children and adolescents, but this website is not directed to children and we do not knowingly collect personal information from children under 16 through it. A parent or guardian should contact us on a child’s behalf. If you believe a child has sent us information through the site, please contact us and we will delete it.
13. Changes to this policy
We will update this page if our practices change, and change the “Last updated” date at the top.
